چکیده

Roentgen stereophotogrammetry was used to measure the migration of the centre of the femoral head in 84 cemented Lubinus SP I hip arthroplasties (58 primary operations, 26 revisions). Four to seven years later, seven femoral components had been revised because of painful loosening. These implants showed greater subsidence, medial migration and posterior migration during the first two postoperative years than did the hips which had not been revised. Six months after operation, subsidence ofmore than 0.33 mm combined with a total migration ofmore than 0.85 mm predicted an increased risk of subsequent revision; the amount ofsubsidence at two years was an even better predictor. The probability of revision was greater than 50% if the subsidence at two years was 1.2 mm or more.

A Single Femoral Component for All Total Hip Replacements Performed by a Trust? Does This Affect Early Clinical and Radiological Outcomes?

Background: Hospitals may be under pressure to implement cost saving strategies regarding prosthesis choice. Thismay involve the use of components which are not the first preference of individual surgeons, or those they have littleexperience with. We aim to examine the effect of standardizing the type of femoral stem used in a single trust, anddetermine whether this is safe practice, particular...

Effect of simulated early weight bearing on micromotion and pullout strength of uncemented distal femoral stems.

The effect of simulated early weight bearing on both micromotion and pullout strength of uncemented distal femoral stems was evaluated in this study. The effect of stem endosteal contact and bone quality on implant pullout strength was also analyzed. A randomized matched-pair study was performed using 8 bilateral pairs of fresh human cadaveric femoral specimens. Each specimen pair was dual-ener...

Simultaneous and multisite measure of micromotion, subsidence and gap to evaluate femoral stem stability.

The initial stability of cementless femoral components is crucial for the long-term success of total hip arthroplasty. This has been reported in animal and clinical studies. Until now, the stability was evaluated by the measurement of relative micromotion on a few simultaneous locations around the stem in cadaveric experiments. This paper presents an extended experimental setup to measure simul...

Primary Stability Recognition of the Newly Designed Cementless Femoral Stem Using Digital Signal Processing

Stress shielding and micromotion are two major issues which determine the success of newly designed cementless femoral stems. The correlation of experimental validation with finite element analysis (FEA) is commonly used to evaluate the stress distribution and fixation stability of the stem within the femoral canal. This paper focused on the applications of feature extraction and pattern recogn...

The effect of surface finish and of vertical ribs on the stability of a cemented femoral stem: an in vitro stair climbing test.

The search for improved femoral fixation in cemented total hip arthroplasty is ongoing. Two design variables, surface finish and stem contour, were evaluated. Sixteen titanium femoral stems of one design were cemented into fiberglass femora. One half of the components had a polished surface and the rest had a roughened finish. Within each group, 4 stems had vertically oriented ribs on the proxi...

The Effect of Physiological Load Configuration on Interface Micromotion in Cementless Femoral Stems

The most commonly reported failure modes of cementless hip stems are loosening and thigh pain; both are attributed to the relative motion at the bone-implant interface due to failure to achieve sufficient primary fixation. Accurate predictions of hip stems’ stability are therefore crucial to the pre-clinical analyses of hip arthroplasty. This study uses finite element technique to analyse the e...
